CAL breast augmentation with SVF | Cosmetic Surgery Times

Does stromal vascular fraction (SVF) affect the retention of grafted fat in patients who have undergone breast augmentation by cell-assisted lipotransfer (CAL)? Noting that CAL may achieve a more natural breast contour than silicone implants and avoid certain complications associated with implants, the authors write that few studies of CAL have looked at long-term volume changes, effects on breast parenchymal tissue and the effect of SVF on graft survival. In an effort to supplement CAL literature researchers — from CHA University School of Medicine and Yonsei University College of Medicine, South Korea — undertook a one-year prospective study of five patients who underwent CAL, with the objective of determining changes in breast volume, effects on parenchymal tissue and the impact of SVF on graft survival. Following preoperative radiologic examination, the five patients underwent primary augmentation mammaplasty by CAL to both breasts. SVF was characterized, and changes in breast volume were determined from MRI readings at three months and one year post-op. Scans were reviewed for breast changes by a breast-imaging specialist. Additionally, an average of 23% of grafted fat in volume was harvested from each patient to isolate SVF cells. The authors found that one year after CAL, breast volume decreased to 47% of initial post-op volume. The only parenchymal change was small oil cysts. There was no correlation found in the ratio of SVF cell count to grafted fat volume. The Stevens laser bra is the by product of a breast reduction technique developed by Dr. Grant Stevens, a Los Angeles cosmetic plastic surgeon. It is primarily created by using aesthetic laser technology similar to what is used on various cosmetic processes to tighten the skin to extend the procedure backwash. The procedure was actually to answer the growing demand for breast reduction with a smaller recovery time. Thus, the operating and recovery time is actually halved compared to the traditional breast reduction surgery. The laser breast reduction is done using a traditional CO2 laser to prep skin as well as the tissue which will be removed during the procedure. Symmastia, or webbed breasts, is a rare congenital breast deformity which results to the webbing of the two breasts right in the middle of the chest, connected via a soft, web-like tissue. This would look like there is only one breast, instead of two. The same effect can also occur if an over-dissection were to happen during breast augmentation surgery. Although the original intention of the operation is to  increase cleavage depth, the implants can end up touching each other in the middle of the chest, crossing over the sternum, resulting in another form of botched breast implants. Other terms coined for symmastia are “bread loafing“, “kissing implants“, and “uni-boob“. Here is how symmastia occurs after an operation. To get the implants closer together to form a cleavage, the surgeon has to release soft tissues that cover the sternum, or the breastbone.
